NCLive REMOTE REGISTRATION FORM

ALBEMARLE REGIONAL LIBRARY

Remote access is obtaining admission to all NCLive resources by an authorized user from a location other than the physical library. It is provided through the use of a password assigned to the ARL by the State Library of North Carolina. This password will permit library users to connect to NCLive resources from home or any other location not on the official list of IP addresses provided to database vendors.

Database vendor licenses permit remote access to all NCLive products. The licenses require NCLive and/or its member libraries to authenticate all remote access users. NCLive IS AVAILABLE ONLY TO INDIVIDUALS; members of an NCLive Community of Interest. AUTHORIZED USERS ARE REGISTERED PUBLIC LIBRARY PATRONS, and faculty, staff and students at academic institutions.

In order to receive access, patrons must read and sign this form. By signing, the patron agrees to, and meets all the requirements of the policy. You will receive a packet of information provided in part by the State Library of North Carolina, which outlines access procedures and guidelines. It is your responsibility to read the information provided to you. Passwords will expire every six months, and the patron has the responsibility of renewing access. Renewal can be done by telephone, by providing the library staff with your name and patron ID number (Library card number).

By signing this form I agree to read the information provided to me and adhere to all policies outlined within:
